How to sign up as an NDIA managed participant?
Step 1
Choose between the 3 options below: Self-managed, Plan-managed or NDIA managed.
Step 4
Once you've signed the agreement, please wait to hear from us for confirmation.
Step 2
Click on the sign-up button, this will redirect you to our web form.
Step 5
After we've confirmed that you have signed the agreement, we will send you a 'Payment Link' via email.
Step 3
Fill out the form, then read through and sign our Disability Service Agreement.
Step 6
Once payment is confirmed, we will send you a 'Welcome' email with the tracking details of your shipped device.
